If you have been selected as a high school player in the All-Ohio Crossroads Baseball Series, you will received notification from your District Coach regarding your selection.

To confirm your place on your district team's roster,

you must print and fill out the Player Profile/Waiver Form below.

We must receive this form ASAP after your selection in order to set the final rosters.
